    There is no special approval process. Since advertising is setup directly between the advertiser and the site owner, we don't need to "approve" anyone since advertisers select the site they want to advertise on themselves.

    Are you sure the JavaScript is there? Looking at the source of the page, it's not there (which is why you wouldn't get any ads served).

    You are trying to run a 4th AdSense ad on a single page. Google won't allow you to run 4 banner ads on a single page.
