How do I get my ads to show like this:?

    That looks like 'AFS Ads-Only'. And if it is, you won't be able to get your ads to look like that unless you have been invited to do so by Google.

    You have to be a premium publisher of adsense to show your ads likes that. For example ezinearticles, articlesbase, etc.

    No you do NOT have to be a Premium Publisher to show ads like that. I showed my ads like that with 'AFS Ads-Only' and I am NOT a Premium Publisher.
