How do I create an ad with a text box?

I've seen ads on many websites where there is a text box for the user to type in something, like a zip code for insurance quotes. I could create the ad itself but I was getting confused on how to advertise it on Adwords. The options I see on Adwords are either text, image, or a display ad builder. None of which, I don't think, could be edited to include a text box. Please help!!

You can not use ads that have input boxes on them on Adwords.

Where could I use these types of ads? .. The ones that I have seen, seem to come from DoubleClick, which is part of Google. Are there any other places where I could use these types of ads?

You should display your web page in several browsers to see how each browser will display your page. There are also some automated ways to create the HTML for you. Have a look at the Resources section for an example of an automated text box generator.
