Google has a good system in place to detect fraud clicks, you shouldnt need to worry about trying to detect them yourself.

Use a ad tracker and if you see that one IP constantly clicking on the ad, ban him..

Adpeeps, not free, but does a pretty good job.

MaxMediaManager, free, and also does quite well; it's just a little harder to use than AdPeeps.

Just download 'em, upload it, select contextual advertising or whatever it is, and put in the google ad code. generate the code and you will see a javascript code to use instead of Google codes and then copy it into the site.
now you can track clicks and all from the administration panel ;)

Isn't it against the TOS to use click tracking software? (modification of the adsense code)

no it's not against the TOS, only google won't comment or recommend any.

By the way I emailed google yesterday because on one of my sites I have been attacked by some lamer for the first time in 2 years and a half that I use adsense, he clicked nearly 400 times (20 dollars) during the day until I removed the ad from this website which does usually 1$ a day.
I emailed them and they'll look into it they said, so I'm going to give them some results when I'll analyse the server log to see this lamer's IP if he used only one.

Some people have nothing better to do with their time.

Whenever my site's been attacked by some lamer, I use my Geo targeting server to track him down through his IP, set my state-of-the-art Missile Launcher upon him, and KABOOM, no more lamer.
