Before you resort to Google Disavow, take the time to launch an outreach campaign to site owners to request that links to your sites are removed. I always recommend a systematic approach. Start a spreadsheet that records the following information:
    • URL/site name
    • Link URL
    • Anchor text
    • URL that it’s linking to
    • Owner name/contact information
    • Records of contacts
Log into Google Webmaster Tools, then go to the Disavow Tool and select your domain.Clicking Disavow Links prompts a menu asking you for a file containing the links you want to disavow. This is the list of any remaining links that you could not get removed manually. Upload the file and you're done.

Just a big old text file. I have seen it work but more often than not, it doesn't work.

The disavow tool doesn't remove backlinks, it only sends Google a signal that you don't vouch for some links pointing to your website, which doesn't make those backlinks go away, of course.
The way to go about it would be to grab a backlinks checker, analyze your links from various perspectives, see which ones could look unnatural to big G, try to remove them and only then disavow the ones you couldn't get removed.

You can't use directly Google disavow tool directly to remove back links. You have to request website owner or webmaster to remove link from their website. If they did not respond you then you can use this tool.
