How Can I Do This .... ??

I am wanting to create the most basic of website, with just a picture of something, then underneath i want a basic contact form:

First Name
Last Name
D.O.B
Address 1
Address 2
State/County
Zip Code

Or something similiar to that .. is their any script which is free which i can just paste in to my sites html and do that?

You could download komposed which is a free html editor, then find a free php email script for the contact form.
