How can I create a paypal subscription form like this one?

    Has a subscription form. All the form asks for is for 2 fields. Name and another field called Steam ID (a gamer's id).

    People enter both information and subscribe on a re-curring subscription. Their script checks monthly if payment was paid and keeps the user's Name/SteamID in the database. It uses IPN obviously.

    Where can I get a subscription form exactly like that one? Enter 2 fields, and subscribe.
    Paypal checks payment on a recurring basis and adds user to mysql database?

    Log into PayPal and click on "merchant services". There is a tutorial on setting up payment forms and recurring subscriptions in there.

    Well I don't see any information there pertaining how to set up exactly like that Free Frag subscription webpage. Could you give me more info on what I have to look at please?
