how can i add a basic opt in form to my home page plz havin lots ofprobs with this

hi all, hope someone can help me, im trying very very hard to put a very basic opt-in form on to my main page, so i can capture first name and email address. Im having so many problems doing what i thought would be quite an easy task! have tried download.com to look for "optin " and "web form" software, but to no avail, ive tried google searches, surely im missing something? is the code that elusive?!

A simply HTML or PHP form, should be quite easy to make... It all depends on what you want it to do with the information, as in whether to email it to you... or add it straight into a MySQL database, or something simmilar.
