Hotel Reservation Form in HTML or PHP

    I would like to ask help. Anyone can give me a link where I can create a reservation form in html or php for a hotel reservation. The form will contain all the necessary information for a booking/reservation.

    How shall I go about this? Thank you for any help.

    You have to use php and mysql, w3schools.com would be a good point to start learning the same.

    I just want to have a ready-made script either html or php, prerably html. I already have a host that runs with php and mysql. All I need is prepared script that I can upload to the site. Thanks.
