Home Page Meta is not changing in wordpress

    I have a website in wordpress when i change it's Home Page Meta Tags (Title or description) on the live website it is changed but when i Google my keyword my website home page is showing in serp but with by default meta title or description. How to fix this issue? Please help me and in some Internal URL portfolio is in the URL showing by default so url looking so ugly.

    Can i remove portfolio from URL if can please provide me any solution.
    URL- /portfolio-items/cras-mollis-nons/
