adding category in the permalink is not a best way. For changing permalinks, WP-admin =>> Settings =>> Permalinks If you want to keep the above structure, then click on custom permalinks and add %category%/%postname%
Yes this has been an issue for many, I remember sorting it out. Though I am not that great with codes, give it a shot. It's been a while, so you might have to research a bit if I am right?! Install Microsoft's URL Rewrite Module, open the web.config file in the wordpress folder and add this, if i remember it write, add it to the system.webServer element - And you'll find the Microsoft URL Rewrite Module here - iis.net/download/URLRewrite - please add htt://www. in front of this link, im not allowed live links yet
