Google "HTML Quiz Tool" to find a ton of sites with these things. Many of them have offsite services for scoring, so you'll need a subscription. But they are usually copy/paste affairs for integration. If you don't want to pay anything, check out the PHP section of Hotscripts.com . They have Quiz and scoring scripts there, but they'll require some PHP coding to integrate - they are not simply copy/paste insertions.
