Help With Coding Auto Insurance Banner to Work on Site - Wordpress - PayPal - ASAP

    Hey, I'm looking to hire someone for a small job. I recently had someone create an auto insurance banner for my site, they sent over a PSD and also JPG file.

    Now I need help coding it so someone can enter their zip code (in the zip code box) and then the "Get Quotes" button will function as needed when clicked. I have the form code needed to work with my affiliate.

    I'll pay $5 PayPal today for some help. PM or reply below.
