Help me to fill the change payee name form:Plz

I want to change my payee name. I filled a form to change my payee name but no change in my payee name. Once more I'm going to send the form.Please help me to fill the form
In the form theres a two questions I have found

Date on which your AdSense account first registered impressions:
Number of impressions registered on your first day:

I'm started my adsense in Jan, previous month I got a pin number and I entered the number. So the month of jan I earn was transferd to this month. But now from the questions, first impression and number of impressions are shown on this month. Which month impression can be answered?Adsense doesnt shown the first impression, only showing this month impression.

I searched that. Where I can find my first impression? When I searched,shown only the impression after the pin entered that mean this month.
