Since you're not making any sense as to WHAT you want it to do, it's hard helping you.
I'm assuming the site runs on some sort of server, which is already most likely PHP or ASP - so what's your point?
Getting an .html-file to run as .php is just changing the file-extension. However there's no point unless you have actual PHP-code in there - PHP runs .html-files just fine (however, you can't process PHP-code in an .html-file unless the server is set up to parse .html-files via the .php-parser).

obviously i'm no programmer.
i just need to have the codes run and throw in details to email.

 

So... what you need is to hire a programmer.

 

first build the form. you can use twitter bootstrap to build a nice form.
When form was completed ask a help how you want to process the data.
Do you want to simply send it to your email or save in database?
