Help: I Wanna Create A Form Like This!

    here is the form i'm trying to create... i'm new to php where could i got to find the information to do this or could someone tell me the steps? also does this need to have a database and if so what would be the best way to create it? thanks

    First off, to securely handle and process credit cards, you need a valid certified ssl server such as openssl mod applied to a http server such as apache or lighthttp. If you don't know what I'm talking about then just give up and use the paypal gateway. They process credit cards for you.
