help fixing adsense error

    I hope someone can help me with this, I have also posted in the main adsense forum but it yielded no response.
    Today i got a red alert bar on my adsense home page telling me i have 'rejected ad requests'.
    The details showed a list of 4 of my ads and said 'The following ad units have made ad requests with incorrect site information. This occurs when the URL of the server from which the ad unit has been served differs from the URL of the actual page where the ad will be displayed'.
    When i looked for more information it said i should change the adsense code of the ad to include the url where my ad is, I would do this but I'm not sure if it's really correct?
    Firstly I thought it was not allowed to manipulate the ad code? and secondly i have those ads spread over a few pages so not sure what to even put in the code?
    Any help would be very much appreciated as I really don't know what to do and am losing revenue.

    are you showcasing a different url in your adsense units than of your site's url?

    Thank you for replying.
    I did recently change my URL's to make them more SEF but I was only changing the sub-domain rather than the main one, that is still the same and did 301 redirects from my old urls.
    My adsense units are not showing any specific url when I look at their code?
    Should i create new adsense units?

    I have attached some screen shots of what the error message looks like. The first one is on the home page, the next is where it takes you after pressing more details and the last screenshot is what google suggest doing which is the bit i'm most unsure about. I've removed any personal information from the screenshots.
