help: Customizable Form in Joomla

    Hi,

    I'm building a hotel website that is powered by Joomla and needs an online reservation. I need is a customizable form that i could customize the fields like (name,address,credit card, rooms, etc.). Is there a plugin, component, or Module in Joomla that exist?

    btw, I'm using both Joomla 1.0 and 1.5 in different sites.
