Header Slider with Video and Lead Gen Form

I see no reason why you couldn't put a form in the content slider version

I'm using RoyalSlider its $20 but wow is it every SMOOTH and amazing, you can throw anything you want in the slides, have a look a their demo video it sold me on the product, i run fullscreen slides in the header and they work amazing on everything from an iPhone to full 1080p screens
