    Hello OvernightGraphics

If you know that your website had been penalized, the first type is always to identify why.
Initially check Google Webmaster Tools and see if you have any new notifications like "unnatural links.

You should then check if there are backlinks which can harm your website.
Over 95% of all Google penalties are in direct proportion to the backling profile of your website.
If the majority of your backlinks are low-quality links, you will eventually lose rankings in Google.

So to recover your rankings go to Webmaster Tools - Search Traffic - Links to your site
See all backlinks and export them. (download latest links)

You can then use a program like Monitor Backlinks to identify harmful backlinks.
You have to manually those verify those that are probably bad.

You can then request removal of the bad links from the appropriate webmaster and then
    Disavow the remaining backlinks.

All in all it's kind of time consuming so be sure that the website does worth the trouble.

    Cheers.

The penalized websites that I worked with, usually had notifications in the GWT because of the unnatural link profile.
In short, you just need to disavow the "harmful" backlinks and after that submit a reconsideration request to Google.
If you repaired all the problems, Google will remove the penalty.
Problem is that this process takes time and if the websites is not very important to you, you might be better of just abandoning it.

It's actually pretty hard. I know it can be done, as HCFGrizzly says, but it's very time consuming and chances are if this is an issue than it's not a website worth fixing in my humble opinion

I guess my approach is similar to the above post:
    1. You need to use Google Webmaster Tools to find all of the links to your website.
    2. You need download the list and usually because there are so many you have to use a tool to identify the bad backlinks (MOZ for example).
    3. Send an email to all those websites asking them to remove the link from your site.
If possible use a system that allows you to track who has opened the email.
    4. For the people who do not reply to your email or remove your link you can use Google Webmaster Tools to remove (Disavow) them.
    5. It's a big pain in the bum...
