    You handle so many sites by building sites that don't require any kind of handholding and setting up a reliable alert system to let you know if something went wrong with a any of the servers, databases or websites.

    Check the sites, 95% and more do not need any kind of updating whatsoever. With a smart design your only work is to monitor the alert log and design your next project.

    As Hex says, I run a cron nightly that checks my DB's for trouble, since that is the most vulnerable, I also try top manually check each site weekly.

    My cron runs every minute for internal network checkup and every 5 minutes for external reach check. I get all alerts on my mobile phone (and email of course) and I feel pretty secured with that.

    Generally, I don't check the sites manually other than when I use them myself or check to see if I want to develop or extend them. I guess I do reach to all of them eventually.
