Supplemental results is Google's way of saying that those pages are not used in the index. In other words, no one will find them during a normal search, (hence the reason you need to include 'url'). Some say it is a bit unfair of Google, others say that it is a way of protecting the results against spam. To get them out of supplemental you need more links to them, (form indexed sites). FFMG

So its not really a bad thing as people think it is. According to google it is an index with fewer restrictions. The guy says he has a solution which works for nearly all sites. But when I did a search on his site http://www.google.com/search?hl=en&q=site:www.1stsearchenginerankings.com&btnG=Google+Search his own pages are marked as supplemental results in the google engine.
supplemental can means lots of things - bad content (dupe content) bad outbound links, can take soem serious re arranging (redoing) of your site to get out, good luck mk

Supplementals are in Google's supplemental index. They do not come up on the top pages for any normal query, unless none of the pages in the regular index manage to meet the required criteria for the search term. So you'd probably see traffic from Google for very obscure terms, and your traffic will take a beating.
another reason is if you move out a URL from your site and google still has it in the list then they would flag it as suplemental
