From what I know google isn't case sensitive. It should be the same.
I have tested this for 2 other search queries and I have noticed some differences between the normal and capitalized form.

Shouldn't matter but we've also seen result sets where it does.

I think it has to do with Google and the way they index sites and advertisements and such. People are always writing in different cases, so I tend to think it does matter, even in the tiniest bit, the case size of the words.

I'm pretty much sure most of the people search for "cosmetic surgery" rather than "Cosmetic Surgery".
In my opinion Google should show the same results, no matter of the capitals.

Here, your site ranked in the same position (14) in both "cosmetic surgery" and "Cosmetic Surgery".

Google started ranking a lil differently. It's interesting to say the least. I wonder when the keyword tools where update to this.

That is weird to say the least since Google, Yahoo and all the major search engines are not case sensitive. I have not faced this situation before, and I have never even heard of it ever occuring. I am really interested to find out what is happening.
