I optimized my website and changed title and meta tags but still google shows old pages, how can I get google re-index my website?

Get some back links pointing to your site and update new sitemap.

Resubmit your sitemap in webmaster tools.

Do SBM for fast crawling again. It helps you.

Things like that happened to me before. It can take some time, but if you follow the advices written above - eventually you'll get indexed for the right pages

I suggest you do any one of the following,
    # try to get quality backlink to your site
    # try to add fresh content to your site or if possible fresh pages also
    # keep doing Social Bookmarking for your site

If you still not getting any results after doing all of these, there will be one chance left in front of you - 'wait for some time..!'

You basically just need to give it some time. Next time google traverses your site, they will get your updates. You just need to make sure google is aware that you have changes, and having a sitemap can help with this. I recently updated one of my sites, and it was until a couple days later that I saw the changes reflected in the search results.

Did you change URL's? If so you will need to redirect the old URL's to the new ones using mod rewrite or equivalent.

Yes, you should check to see when the last time the search engine bot visited your site and see if there are any trend in your history as to when you might expect it next. The same thing happened to me and it took about 2 weeks to update. Good luck!
