Google Images Optimization Question

When searching something out with Google Images, you will notice that some of the results will have keywords bolded underneath, while others don't but still show the right type of images, for the most part. What I would like to know is what counts most when trying to optimize your images for Google (if the following count at all that is):

    - The Title of the page the image is displayed on
    - The Alt tag on the image
    - The text on the page the image is displayed on and how well optimized it is for that keyword
    - The name of the image file Ex:cute-puppies.jpg
    - Keyworded content surrounding the image

Please let me know if you know, thanks :)

The only two things that have related to my images that were indexed by Google Images were the filenames and the alt tag text.

My photo gallery initially was getting none of my photos indexed by Google. I am using Coppermine, and it was just putting the filenames in the alt tags. Most of the time, my filenames are just the numerical filenames generated by my digital camera. Some of the few images that I had renamed to something relating to the subject did get indexed by Google images, the rest did not.

I modified Coppermine to put the description in the alt tags. After doing this many more of my images got indexed by Google images, so apparently the alt tags count heavily.

None of the text in the description associated with them got indexed, so apparently that does not count for much.

Under the best of circumstances, Google Images takes months to years to index new content, as opposed to text content which can be indexed very quickly. I don't know if they have a human reviewer check each picture to make sure it's not illegal or the hold up is but it's a very slow process compared to text indexing.

I doubt there's a human reviewer, since if you search, you'll notice that a lot of pictures have doubles from various websites (some coprighted).
