That's wrong. My website was banned for being "adult" though my Adsense account remained. I then appealed and the site was unbanned since it is not actually adult.

Yes 100%, in my research Google not ban old accounts, they only ban websites for content related problems. But they ban whole account for invalid clicks.

Many of the pages were not even close to being adult yet they banned the whole site- "ACTION TAKEN: We have disabled ad serving to your site."

That's correct. My Adsense account was not affected and I continued earning money on my other site and YouTube partner channel. I then appealed against the decision to disable ads on the first site and they immediately apologised and enabled them again.

Always follow the AdSense TOS. Google is very strict about enforcing it.

Yes but rules are open to interpretation. For example a website about racism may be seen as racist if a reviewer only scans through it quickly. Just as mine about red light districts was at first deemed to be breaking the rules when it clearly doesn't-

    "VIOLATION(S) FOUND:
ADULT ESCORTS OR DATING: As stated in our program policies, AdSense
publishers are not permitted to place Google ads on pages with adult
content. This includes adult-oriented dating and companionship services
    such as mail-order brides. "
