I think when you want higher CTR then you've optimize your content according to location. So if you want visitors from America, you must mention United States of America somewhere within your text on your blog / site. That's how you can get more clicks on your adsense from specific countries. I liked the post but you forgot to mention the reason other than recession as to why google adsense CTR of most publishers went down. I think google changed its policies.
That may have a simple affect but when you think of it you won't be able to write articles for cretin countries + Google Adsense is a very developed program they show the Ads according to your country, example if you're surfing a hotel website they will try to show the hotel ads in your country if there's any,
