I always thought it was a combination of CTR and the approval of an AdWords staff member...

    AdWords Help Files say:

For the top positions above Google search results, we use a slightly different formula. First, only ads that exceed a certain Quality Score threshold may appear in these positions. Second, for ads that do surpass this Quality Score threshold, we use the actual CPC rather than the maximum CPC when determining the ad position for these spots. This ensures that Quality Score plays an even more important role in determining the ads that show in these positions.

By adjusting your keywords' Quality Scores and maximum CPC bids, you can better control the position of your ad. Remember: The higher the quality, the lower the CPC, and vice versa.

it do not depends of the cpc, I already got it paying $0.06 per click - yes it depends of the competition of the keyword.

To get the "Top blue shadowed sponsored links", you need to have:

    - a very good Quality Score history
    - Your ad need to be already approved (remember that for search network, your ads works normally even not manually approved yet by a revisor)
    - the search needs to show not only your ad but a lot of ads including competitor ads (is not common to see a search showing only top blue sponsored links and no left sponsored links)

So this is very important term. If your site targets what keyword you are using for adword then it may have slightly higher chance of getting in blue box.

Its a bit like saying how much is it to buy a car, it depends on a lot of factors. Do your research to get a better idea of costs.

Absolutely - you can get a fair impression by starting to set up an adgroup, with the keyword that you are interested in in, and try different cost per clicks to see at what point it predicts that the number of clicks levels out.

Then close the adgroup without saving it.

you can set your position preference in your keywords tab to 1-3, 2-10 etc...
however if you want #1 it costs. I am working on ads for a company and it was expensive to hold #1 but you can do it if you set position preference to 1 and bid high.
