to do it from scratch you will need to know PHP, HTML and CSS as well as have the design. You can alter the free templates if you so wish to practice with at the start but if you are creating your own then there are step by step guides to how to make them.
As there are some many free and paid themes available, it would be difficult starting from scratch to break into this market. You should check out the websites for paid themes to see what is being offered and if you can better that.
