    What qualifies as not being Adsense safe? As far as I know, it's fine.

    15 seconds initially, but longer if your site interests them.

    Could be from 50-500 daily if all countries are allowed. There is an option to have certain countries view the ad, if that's the case it would probably be 10-250 per day or so, until the ad package has run out.

    Yes, you can have USA only. It may just take a little longer to receive all the traffic, but you will continue receiving the traffic until you have got what you paid for.

    USA only traffic is an additional $0.50 so $3.50 total for every 5k hits wanted.
