I have no idea about prizes, but for design its look interesting.
great photo, look more eye catching

 

I really like the design, of course you'll have to change the header logo match your website's name, but that's about the only change I recommend design wise.

As for getting people to complete surveys for you, you might try offering coupons in exchange (hosting and domain).

 

Yeah, the design looks okay, but your success is going to depend on these "freebies" and the purpose of collecting the surveys. Do you have a rough plan of what you want to do?
