    Where can I get a free web form html code?

    I need the form to send email to "support@mydomain.com" with the subject line "subscribe ezine" & body text "First Name : Last Name"

    That's all! As simple as it can get!

    Your best bet would be to learn it (with a little dedication its pretty easy) so you wont have to rely on any resources online to generate it.
