Form Script that dumps inputs into database script

I'm looking for a simple script that lets me create many form fields (I'll need upwards of 20), then when the user hits submit on the form, the results get emailed and posted to a database on the server. I would also like the ability to directly enter and edit records into this database and have password login for multiple users.
