Form Needed Please

i need a form that let the visitors put their:
Email
Gender
Age
Country
telephone
Mobile

and after submitting these information i got notified with an email .

I'm not into programming, but I think there's a lot of sites where you can achieve this. For example:

That will give you a good start. The only problem I see with that form generator is that your email is exposed.
