Form creation script

    1. I need a form creation script, where I can add name, dob, etc. (see attachment)

    2. These fields need to fit in the 8.5X11 landscape size

    4. See attachment for the setup. I am not sure about the exact placements, therefore, I need to be able to fix that, since I do not have an actual copy of the form in my hand.

    What script language are you looking for? web based (php) or stand alone (VB)?
