Filing a reinclusion request

My site has disappeared from search engine results. It appears that it's still indexed but just doesn't appear in google search results anymore. Previously I was getting a few hundred visitors via organic search. Now I'm lucky to get 20.
Does anyone know where you go to file a google reinclusion request?
    Cheers

it's totally useless and a waste of time!
I don't even thing they read them since they probably get thousands of requests every day. You also HAVE to agree that you did something wrong!!!!!! otherwise you can't send the request.

I read somewhere that when someone send a request, there is a bell that rings in the break room in the Plex so they can have a good laugh every time a request comes in. Not sire it's true though ;)

    Its in Google Webmasters tools..
    It is not a waste..
I have an experience with this.
Once google blocked my site with SERps because I violate google rules and regulations..
Then I correct those things and request to google to reindex my site..
    Now its ok...
After 2 or three weeks it came in SERPs

    This is the link
