File Versions?

When a file says v1.4.3 etc., what do all the numbers mean? I understand that the jump from 4.2 → 5.0 is bigger than the jump from 3.1 → 3.2, and I also understand that the numbers represent increasingly smaller changes, however I'm unclear as to what the standard is for deciding what physical change constitutes what numerical change. Is there one?

    Thanks for the help,
    Peter
