File upload script required

Hi, My client is seeking new staff and wants me to add a CV upload facility on his site

Its just a small HTML site with a contact form, at the bottom of the form he wants potential staff to fill in their details and then be able to browse their computer and upload their CV

Anyone got something that will work?
