I want to create a form where will be some field as the snapshot and also have the File upload option (Only *doc extension File).If anyone upload *doc file and fill up the form then the file will store in server and a notification with the filled information will be send to administrator via email .Is it possible?

Below i have mentioned a form i created, but when i Press Submit it actually refresh the page instead of uploading the file to server and sending email with the filled form information. I want this form to work like: Whenever someone fill up the form and upload a file it will be emailed to a particular email address and the file will be saved to a particular folder. Can anyone help me with the code?
