How can I make feedback form like this? For HTML site?

What do you want the action to do with the submitted information?

 

Those information should automatically send to a e-mail. In this case my e-mail.

I don't know. Anything easy for u is OK. I need to use this with HTML website.

You need to determine what your host has available: either PHP or ASP. If none are available, you may need to fall back to a CGI script to handle the mailer so check with your host.

 

I have PHP support in my host (Servage.net)

 

Do you need just HTML and CSS markup of that form or PHP code that will send the feedback?

If you run Wordpress, you can use wp_mail() instead: http://codex.wordpress.org/Function_Reference/wp_mail
