Is it a good on-page seo practice to give exactly same keywords phrase in URL of the post, Title of the post, Meta-description of the post & alt. text to image in the post.

The three most critical places to have your target keywords are the:
    1. Title
    2. URL
    3. Description
As a general rule of thumb, and what's been working for me well since 2013 standards have changed, is to use the exact keywords only 2 out of these 3 places. Then use a synonym for the third slot.

    Example: dog training
    1. Learn the best dog training techniques
    2. site.com/dog-training
    3. Check out our canine obedience tips.

Well... It's not a very good idea to use the SAME keyword in ALL possible places. Mix them up as @DiggitySEO said if the word is dog training, use:

good dog training tips for a title
dog-training-tricks for an URL for example
dog training techniques for a H1 tag

Use different as this will help you get a better ranking for all of them. Then you need to also do a quality work on your Off-Page SEO to really achieve this! ;)

I would also use similar kind of keywords (Latent Semantic Indexing) to be used in the post for better ranking.
