You're probably including the php file twice. A redeclare error happens when a "function function_name_x" happens twice. That's not allowed.
Wow, this was just driving me crazy. I had been trying to set up the code on a new site... started out at 1:00 a.m.... it is now 6:21 a.m. but I finally got it working I just hate leaving things unfinished... even if it means that I have to stay up until the sun starts going out Oh, well, I guess I'll go get some sleep now.
