Buying email submission form

    Hi,

    I am looking for someone to make and install a script that enables by visitors to type in their email address and name in a field, and hit a "Subscribe" button - Then, I want my email to recognize the submitter's name, and auto-respond to him (Such as: Hi [name]! etc) with a predetermined message. Something like this:
