EASY Image Design Contest - 24 Hours $15

This contest should be pretty easy so I limited it to 24 hours. I need a simple image for one of my websites. The image needs to look like a form for entering information to choose a college. A smaller version can be seen HERE (and below). The image size I need for this website is 471x213. Be creative and add fields if necessary but again I ONLY NEED THE IMAGE OF A FORM ... NOT AN ACTUAL FORM. This contest will end tomorrow (Friday) at 3pm PST. Winner will be paid immediately via Paypal.

buncoh - nice design but I am not crazy about the image used. Can you try something else?

macgfx - I like the concept but the color is too dull

 

This contest ends in a few hours. If I do not receive more entries I will extend for 24 hours. This should be easy so I am unsure why I have only gotten 2 so far!
