No content? Pseudo-filesharing? Can you duplicate that page on another domain and see if the ads display there, if they do, the site is the issue, if not, then it could be something else. Like I said, it displayed fine here.

I think your site url is only banned. Can you access your adsense account? If then that domain is banned. It might be banned because of adult images most probably.

If you checked the IDENTICAL code and the ads show up fine on another domain, about the only alternative is the site has been banned. Like I said, probably either because the page has no real content on it, it's a file-sharing site, or as grandy stated, someone may have uploaded adult image which will get you banned in a hurry.

Could have saved a whole lot of time if you had mentioned that in your opening post :)

Did you inform them that you had removed the material? Did someone repost? What was the nature of the email, was it a ban or a warning telling you to remove it?

There's the problem. You need to contact them within 3 days of when you get the email to let them know that you've corrected the problem. Then they can check the site to be sure you are in compliance. Since you didn't do that, I can only assume that they pulled the plug on that site. Was the email a warning or a ban? If a ban, you were probably out of luck from the start, but if it was a warning, you should have responded within the 3 days saying you had corrected the issue and please look at the site again etc.

    1) Put analytics tracking code where its supposed to go.
    2) Copy your site to another directory and try there.
    3) Mail adsense with reference to some people seeing it and some not.

Try it out. It might work, might not, but if you don't want to try then we can't really help you.

    Edit:

Seems you have pinpointed the error.

    Good luck.

I think we've narrowed down the problem. Doesn't look like it's code.

Yes. Adpubster is right. When you received that mail, you should always send a reply saying you cleared everything which was violating copyrights and ask them check again for any issues. in that case, you are safe. If you dont reply, they take it as you ignored their notice.
