Normally you purchase a domain for a set period (ie: one year). The renewal time will be at the end of that set period (ie: at the end of one year from the purchase date). Renewal times can be the same for all domains that you buy or they can be different dates for each domain depending on the dates that you bought them.

If I understand you correctly that would be the choice of the individual registrar. I'm not sure if ICAN has any rules for expiration periods. I haven't personally looked at buying any kind of extension though. I normally just renew each domain that I own for a minimum period of one year when renewal comes due. Maybe someone else here can answer your questions.
