Does This Type of Form Exist?

    Hi,

    Apologies in advance if this is quite a vague question, but I wonder if any of you can help. I've got an idea for my company's webpage which will give us an edge on our competition, and I'd like to know if such a solution already exists; if so, what is it called?

    I need some sort of form that will be sent from one of our members to another. The information must be viewable to the sender, receiver and also the webmaster; once the form has been sent, it cannot be edited. The "form page" which will have around 20/30 editable fields and will sit in the password protected part of the site, as all the information will be private.

    The above will replace a document which all members currently have to complete when doing business with each other; I'd like to make it easier for them to pass this information between themselves - at the moment, it's a case of printing the document out, filling it in, scanning and then emailing it to the other member.

    Now, I'm not sure whether this system will sit entirely in the password protected area (i.e. the form will be sent from one member's profile page to another's) or whether it will generate a fixed, HTML email that will be sent out directly from one to the other. This will all be decided later.

    If anyone can give me suggestions for the above I'd be very appreciative. I need specific details with which to go to our design team.

    It will be possible look into PHP forms as you can set them to email whoever say for instance it takes the recipient and senders email from the form and emails the message to them and coded in the page it will send to the webmasters email too.
    It is possible, kind of like the pop up boxes you get on job sites to send jobs to friends.
