Do the TOP ads need to be approved by Google?

Hello,

My ads used to show at the TOP of the results, so the CTR was really
high. Now for some reason, I edited lot of factors using the Adwords
Editor, uploaded the changes, and now the ads dont show anymore at the
TOP of the results, so the CTR has been divided by 10... When I look
at the ads, I can see that the old ads were deleted, to be replaced
with the exact same ones.

So, I was wondering, if Google needs to approve an ad, in order to
show in the TOP results, or is this only based on CTR, CPC,
competition and Quality?
