    Hi people,

    For a rusty old project that i'm starting back up i'm looking for someone that is handy with styling forms, as i have a large survey style form (one page) that could have some TLC.

    Can't post the form here as it contains adult texts (it does not contains photos, links or bad stuff) but when interessted i can send you an example by private message.

    Describing the form: It contains a personal part, small medical part and over 400 same type questions withing a couple of categories.

    Before you send me a private mesasge, also think about estimated price in EUR/DOLLAR and send me some examples of your works.
