I need a header, a footer and (html) table design for a product of mine. The product is a webtool, so it looks like a website. The theme is quiz and survey related. I have no high demands, except that it shouldn't look to 'kid-like' (no picture of an old guy looking confused with a '?' above his head). Usually I'm not too fond of photo's in the header (except when it's done really well). <EDIT: No more replies please, I got sufficient replies. Thanks everyone!>
